Updated TDAP form required for all CPSD seventh grade students

The State of Mississippi requires all students entering the 7th grade to provide proof of an updated TDAP (tetanus, diphtheria and pertussis) booster. A recent TDAP booster is defined as being administered after the age of seven.

 

The Clinton Public School District requires a physical copy to be presented to school officials prior to the start of school. For an example of the correct form, visit THIS LINK.

 

Parents and guardians can present a physical copy of the updated 121 Vaccination form when proving residency on July 20-22 and July 25-26, or they can present their physical copy to CJHS officials during Meet the Teacher Day on August 4.

 

“This step is only required for students entering 7th grade,” CJHS Principal Tamikia Billings said. “Once a student has received his or her TDAP booster, parents can bring the updated vaccination record when they prove residency or they can bring it to the CJHS office prior to the first day of school.”

 

Anyone needing to locate an updated 121 Vaccination Form can contact their pediatrician, the Mississippi Department of Health or get access to the MyIR Online Database with MSDH at msdh.ms.gov.
