Parents need to fill out the yellow registration form, the home language survey and the transportation form, all of which are available at the school. They should also bring:

• Original long form birth certificate

• Original Social Security card of child

• MS Immunization Form 121 — checked complete for school entry

• Two proofs of residency:

-       Lease or mortgage document is required. The lease must include the name of the child, parent’s name and address. The lease should also include the length of stay and expiration date. We do not accept mortgage payment booklets. You may also use your 2011 Homestead Exemption Form.

-       Current utility bill (gas, water or electric — within the last three months)

• A photo ID or driver’s license of the parent or guardian registering the child.
